Empowering Veteran Service Providers Across the Nation

PATRIOTlink is a network of vetted and trusted public and private organizations across the United States developed by COSF. Our Case Coordination team leverages organizations from PATRIOTlink to increase our capacity to help service members, veterans and their families.  The estimated 30% of veteran families in crisis typically require assistance from a service provider or case manager to ensure they have wrap-around support to facilitate their long term well being.

We recognized that if we could scale the network and our program model of holistic Case Coordination, we could empower all service providers to collaborate more effectively and have a greater collective impact than any one organization is capable of alone.

So COSF developed PATRIOTlink, a cloud-based resource navigation platform designed by service providers for service providers. The strategically populated provider assistance tool empowers service providers to easily navigate and find resources for the service members, veterans and families they are assisting. Our team of PATRIOTlink Program Specialists perform a thorough vetting and profiling process on each active resource, spending approximately 90 minutes per organization. They ensure that each profile represents program capacity, includes services provided and is tagged based on eligibility criteria (i.e. service era, disability rating, discharge status, deployment history, population assisted and geographic coverage). This allows a service provider to perform searches that result in resources their client qualifies for, significantly reducing the amount of time it takes to find the often multiple resources their client needs.
